Oncology means the study of cancer, medically termed as
malignant neoplasms. Specialists who have primary medical interest in cancer
are called oncologists. And Surgical Oncology is a special branch of surgery
which deals with the planned and coordinated surgical care of the cancer
patient. It is the most common and oldest form of cancer treatment in which
treatment is mainly done by removing the tumors or the affected tissues.
The practice of surgical oncology is mainly based on the good and
sufficient knowledge of basic tumor biology and the natural history of
various cancers, their effects or combinations of different treatment
procedures. Society of surgical oncology has approved fellowship for 14
types of surgical oncology. And the surgeons who have completed any one of
the fellowship are recognized as the surgical oncologists.
A surgical oncologist is trained in the principles of multidisciplinary
care and multi-modality treatment options. They are broadly trained in all
forms of oncology, but specializes in one of three main disciplines :
Surgical oncology - cancer surgery
Surgery plays the most important role in cancer treatment. It is the most
common and oldet form of cancer treatment. The various disciplines in which
surgical oncology can be undertaken are Breast, ENT, Gynaecology, Gastro
Intestinal Surgery, Head & Neck, Orthopedic, Peripheral Surgery,
Thoracic, Urology, Plastic Reconstructions
Radiation oncology - radiation therapy
It is an important approach for cancer treatment. In which treatment is
mostly done with the ionizing radiation. The cases where it is not possible
to operate, radiation oncology plays a pivotal role in curing the disease.
It is often recommended for the local control.
Medical oncology - chemotherapy/hormone therapy
It is a sub-specialty of oncology which deals with the medical management
of cancer. A medical oncologist is fully trained in all types of cancer
treatments including chemotherapy, radiation, surgery and biological
therapy. But medical oncology mainly comprises of chemotherapy which means
treatment of cancer with the help of anticancer drugs. These are given to
the body in the form of injection or tablets. Sometime chemotherapy
treatments has serious side effects also. So it is essential to perform this
treatment with utmost care.
But as different types of cancer behave very differently, so surgical
oncologists are usually specialize in a particular type of cancer or areas
of cancer treatment. The training, expertise and practice of a surgical
oncologist often sub-specialize in organ groups or regions (eg breast, upper
GI, melanoma or colorectal surgery) or across several tumor types (eg
melanoma, sarcoma and breast).
Surgical oncology can be further classified into four forms :
- Preventive surgery- Preventive surgery is require to remove the
nonmalignant body tissue before it is likely to become aggressive and
malignant.
- Diagnostic Surgery- It is usually performed through a biopsy, to
obtain a tissue sample to determine whether it is cancerous or not.
- Staging Surgery- Staging surgery is performed to evaluate the extent,
size and type of cancer.
- Palliative surgery- Palliative surgery is essential in relieving pain
and other complications caused by advanced cancer.
