The medical term for impotence is Erectile dysfunction. It is a
sexual dysfunction found in male only. In this, the patient is unable to
develop or preserve an erection of the penis firm enough for sexual
intercourse in spite of the ability to ejaculate. Other problems which can
interfere with the sexual intercourse and reproduction are also related to
this dysfunction. Such as lack of sexual desire and sometimes problem of
ejaculation or orgasm.
Erectile dysfunction or ED can be divided into three types such as :
- complete inability to achieve erection,
- an inconsistent ability to achieve erection and ejaculation,
- a tendency to maintain only brief erections.
These three variations has created problem in defining and estimating
its incidence.
Reasons behind Erectile Dysfunction :
- The main reason behind erectile dysfunction is any kind of injury to
the penis or area near the penis which includes nerves, smooth muscles,
arteries, and fibrous tissues of the corpora cavernosa. Even injury to
nerves and impairs blood flow in the penis can also cause ED.
- Even injury to the spinal cord, prostate, bladder, and pelvis can
lead to ED.
- Psychological, physiological and habitual reasons-Person suffering
from erectile dysfunction can have a history of causes such as
physiological such as diabetes mellitus, kidney disease, multiple
sclerosis, atherosclerosis, vascular disease and neurological diseases
or habitual causes like chronic alcoholism, chain smoking, overweight
and avoid of regular exercising. Psychological factors such as stress,
anxiety, guilt, depression, low self-esteem, and fear of sexual failure
can also causes 10 to 20 percent of ED cases. Psychological impotence
can often be helped by approximately anything that the patient believes
in.
- Even some surgery like radical prostrate and bladder surgery for
cancer which sometimes injures the nerves and arteries near the penis
also can lead to ED.
- Many common medicines such as antidepressants, antihistamines,
tranquilizers, appetite suppressants, blood pressure drugs and
cimetidine (an ulcer drug) can also lead ED as a side effect of these
medicines.
Older man mostly suffers from erectile dysfunction due to physical
causes only such as injury or side effects of drugs. This dysfunction
increases with the age. Approximately 5% of men above 40 years of age and 15
to 25% of 65 years old men suffers from this disorder. But it can be treated
at any age.
Techniques to diagnosed Erectile Dysfunction
Patient History : The main techniques to diagnosed the ED is to know
the medical and sexual history of the patient which is a great help in
defining the degree and nature of ED. A medical history helps in disclosing
the diseases that are root cause behind the ED whereas a sexual history
helps in identify the main reason behind the ED by distinguishing among the
causes like sexual desire, erection, ejaculation.
Physical Examination : A physical examination of the patient can
provide information about the systemic problems. For instance, if the
patient is not feeling any sensation in his penis after touching it, than
there must be a problem in the nervous system. Hair pattern or breast
enlargement which comprises abnormal secondary sex characteristics, can
point to hormonal problems, which means the involvement of endocrine system.
The doctor might realize a circulatory problem in the patient by observing
reduced pulse rates in the wrist or ankles. A penis that gets bend or curve
while erection could be the result of Peyronie's disease. It is an unusual
characteristics of the penis which could also be the main cause of the
problem.
Laboratory Tests : Several laboratory tests are also performed to
diagnose ED. Blood counts, urinalysis, lipid profile, and measurements of
creatinine and liver enzymes are some of the tests to detect systematic
diseases. Measuring the amount of free testosterone in the blood can help in
getting information about problems with the endocrine system and is
indicated especially in patients with less sexual desire.
Psychosocial Examination : A psychosocial examination includes face
to face interview with the patient and a filled questionnaire by the patient
itself which helps in identifying the various psychological factors involve
in the disorder. The sexual partner of a man can also be interviewed to find
out the expectations and perceptions during sexual intercourse.
Treatments of ED
After identification of the root cause of the disorder the treatment is
started.
- If the disorder is due to the habitual reasons like smoking,
alcoholism or obesity than the doctor prescribes the patient to change
their habits. And indulge themselves in some healthy life style tips to
regain sexual ability.
- If the disorder is due to the side effects of some drugs than ask
your doctor to immediately change the medicine.
- Psychotherapy- ED occurs due to psychological reasons can be treated
using techniques that decrease the anxiety associated with intercourse.
The patient's partner is a great help in the techniques. The partner can
gradually increase the level of intimacy and stimulation. This technique
can also be used in relieving the anxiety occur due to physical causes.
- Drug Therapy-In this therapy drugs are taken orally or injected
directly into the penis or inserted into the urethra at the tip of the
penis to treat ED.
- Mechanical Vacuum Device is an instrument that helps in erection by
creating a partial vacuum, which draws blood into the penis, engorging
and expanding it. This device has mainly three components a plastic
cylinder, into which the penis is placed, a pump, which draws air out of
the cylinder and an elastic band, which is placed around the base of the
penis to maintain the erection after the cylinder is removed and during
intercourse by preventing blood from flowing back into the body.
- Surgery can also be used to treat this disorder. It is mainly done
to achieve these three goals such as to implant a device that can cause
the penis to become erect, to reconstruct arteries to increase flow of
blood to the penis and to block off veins that allow blood to leak from
the penile tissues.
